What You’ll Do

As a vital member of our Integrated Neighbourhood Team, you’ll:

  • Deliver high-quality, person-centred care to patients in their own homes and residential settings
  • Work closely with GP practices and local community services to prevent unnecessary hospital admissions
  • Support patients in managing their conditions independently within their own environment
  • Collaborate with Nurses, Therapists, Clinical Leads, the Home First Team, and Care Coordinators

We’re seeking inspirational Registered Nurses who are:

  • Clinically skilled with up-to-date professional development
  • Compassionate communicators who can build trust and rapport with patients and families
  • Resilient and adaptable, able to manage a busy caseload while maintaining high standards of care
  • Experienced in front-line NHS community or acute care (essential)
  • Motivated team players eager to contribute to the development of our vital community services
