Are you looking for an exciting new opportunity? We are recruiting a part time Care Coordinator to work as part of our Multi-disciplinary Team!

Based at our Care Coordination Centre in Peasedown St John, BaNES, you’ll join a supportive non-clinical team of Administrators and Care Coordinators, working together to provide outstanding service to our patients and clinical teams. Collaborating with Duty Clinicians, the Community Wellbeing Hub, Social Care, and Integrated Care Teams, you’ll play a vital role as the first point of contact, delivering information, advice, and signposting, as well as managing referrals, appointments, and patient correspondence for our community healthcare services.
This is a part time opportunity working 22.5 hours across the week based in our Peasedown St John Care Coordination Centre. The Centre is currently open 08:00 until 20:00 Monday to Friday and 09:00 until 17:00 Saturday and Sunday. The post holder will be expected to support these extended working hours on a rota basis.
Main Responsibilities
- Handle first-line telephone calls professionally, transferring or directing them as needed.
- Process and accurately record referrals, ensuring they are directed to the appropriate professionals.
- Schedule, reschedule, and manage clinical appointments, maintaining adherence to protocols.
- Respond to shared email inbox requests and manage patient correspondence, including sending appointment letters.
- Provide a signposting service, guiding patients and referrers to the appropriate services efficiently.
- Maintain accurate records and adhere to all guidelines, policies, and procedures.

The Ideal Candidate
- Experience interacting with patients and/or customers.
- Proficient in IT, including Windows applications and Microsoft Office, with the ability to learn clinical IT systems.
- Clear, concise, and professional communication skills.
- Team player with the ability to liaise effectively with individuals at all levels.
- Strong organisational skills and the ability to assess risk with discretion.
- Driven to deliver high-quality service.
Experience in a medical practice or Healthcare environment would be beneficial for this role.
